138 by PowerPyx's count: 85 clues and documents, then 13 phone numbers, 15 strange postcards, 18 CRTV signals, 7 resource containers. The last four are listed below in a checklist, chapter by chapter, because the game keeps no clear record of which ones you already have.

You cannot go back. Areas close behind you and there is no chapter select, so everything here is missable in one run. Reloading a save before the spot, or a new playthrough, is the only way back.
Progress carries over. What you collect counts across playthroughs, so a second run can pick up what the first missed. Postcards you already have do not reappear in New Game Plus.
Only documents change the ending. The 85 clues and documents are the Discoveries the game weighs against your kills; the other four types only matter for their trophies. How that weighing works is on the endings page.
One square per item in the checklist below, by the part it sits in. Part 4 holds the most; a dashed square is one only one guide lists. Original chart for this site.

Ticks stay in this browser only. The game keeps no clear record of which of these you already have.
Most numbers only count once you have sat through the whole call until it hangs up on its own. Most have to be called where you find them and as the character who finds them; the council’s number, for example, will not connect unless you are playing Zoe, and Zoe’s own number has to be called as Zoe to set off her pager. Two of the thirteen come from other calls: 946 hands you one number in chapter 3 and another in chapter 10. The list in the phone menu only shows numbers the current character can use, so it is not a full record. Per PowerPyx: all phone numbers
Postcards are a puzzle key
The pictures on the fifteen postcards spell out the order of the buttons on a locked hotel door that only matters in New Game Plus, for one of its two extra endings. You do not need the postcards to open it; the order is drawn out on the New Game Plus page. Per PowerPyx: all endings guide
Signals and radios
Most signals are free-floating channels: open the handheld television, sweep to the number and store it while the tracking dot sits in the box. The ones at radios are a small puzzle instead: steady the picture with the tilt, then move the coloured shapes into their boxes. PowerPyx counts 18 signals; Game Rant counts 19, adding a box in the security room, marked in the list. Checking it costs nothing if you are going for the trophy. What the handheld television is for is on the CRTV page. Per PowerPyx: all CRTV signals · Extra box from Game Rant: all CRTV signal locations
Containers need their key first
Each of the seven resource containers has its own lockbox key, always found in the same chapter and usually close by, so every row above names both. Two of them sit in Clara Woods, where the codes for the lockers and safes are on the codes page. Per PowerPyx: all resource containers
Where these fit
Chapter numbers match the chapters page. Every trophy named here, with the rest of the list and a separate checklist, is on the trophies page, and the puzzles you pass on the way are solved on the puzzles page.
